What is Remote Therapeutic Monitoring?
Remote Therapeutic Monitoring (RTM) is a family of five codes created by the CPT Editorial Panel
in October 2020. Medical necessity is required. Patients must have a minimum of one diagnosed chronic health condition. The correct ICD-10-CM code (diagnosis) must be assigned to submit Medicare claims for remote monitoring. The justification for RPM should also be documented in the medical record to be compliant.
Patient consent: must be obtained before providing RTM services.
RTM encounters of care begin when remote monitoring of a specific condition or treatment goal is initiated. Services end when the targeted data is collected and/or treatment goals are met. Devices used to monitor responses to interventions do not have to be approved by the U.S. Food and Drug Administration (FDA). The smart watch is an example of a monitoring device not approved as a medical device by the FDA. Smart watches can measure activity and sleep.
Comparing RPM and RTM
Outpatient therapists use RTM to collect information about the patient’s response to therapy, adherence to the
therapeutic regimen, musculoskeletal or respiratory symptoms experienced, and other non-physiologically
information. The patient self-reports this data, which the provider enters into the electronic medical record.
These services are different from those provided with remote physiological monitoring (RPM), which collects and transmits vital signs and related information with electronic technology. While RPM billing requires the use of Bluetooth Technology, RTM billing does not. Instead, it must use a web-based platform classified as software as a medical device (SAMD), which surveys patients about symptoms, medication adherence, pain levels, and other aspects of treatment and outcomes.
