About this course
This course covers the process of identifying the most accurate list of all medications that the patient is taking, including name, dosage, frequency, and route, by comparing the medical record with to an external list of medications obtained from a patient, hospital or other provider. The Joint Commission defines Medication Reconciliation as a formal process for creating the most complete and accurate list possible of a patient’s current medications and comparing the list to those in the patient record or medication orders.
Objectives
- Introduce the background of Medication Reconciliation and the role of CMS regulations.
- Make clear the definition of medication errors, causes, classifications, types, contributing factors.
- Recognize safety vulnerabilities of medication errors.
- Identify Medication reconciliation process to prevent, minimize and address medication errors.
- Develop basic knowledge to submit Medicare Part B claims for Medication Reconciliation
- Develop a general understanding of professional and legal consequences resulting from medication errors.
Who is this course for
This course is for nurses and case managers. Two (2) Continuing Education Units (CE) are earned once this course is completed.
